Is hazmat cleanup the same as biohazard cleanup?

No. Biohazard work is blood and other potentially infectious material under the bloodborne pathogens standard. Hazmat work is chemical and hazardous waste under the hazardous waste standard and the waste rules, and the two carry different training, waste routes and prices.

Who decides whether the waste is hazardous?

The generator does, which in practice means the owner or occupier acting on the contractor's determination. It is not optional and it is not a formality, because the answer decides the transport, the destination and most of the cost.

Can ordinary waste collection take any of it?
